If you have one of the following medical conditions you may be entitled to prescription charge exemption. Please ask at Reception for a Form FP92A.
- A permanent fistula
- Epilepsy for which continuous anti-convulsive therapy is required
- Diabetes mellitus and treatment is not just by diet alone
- Myxoedema (hypothyroidism requiring thyroid hormone replacement)
- Hypoparathyroidism
- Diabetes insipidus or other forms of hypopituitarism
- Forms of hypoadrenalism (including Addison’s disease) for which specific substitution therapy is essential
- Myasthenia Gravis
- A continuing physical disability which means you cannot go out without the help of another person
- Undergoing cancer treatment
